Before Your Appointment

Insurance benefits vary by plan. When you submit a Request an Appointment form, you will be asked to provide your insurance information so our office can assist with checking your eligibility and estimated behavioral health benefits before your appointment.

Please note that eligibility and benefit information provided before a visit is an estimate and does not guarantee coverage or payment. Final coverage and patient responsibility are determined by your insurance plan when the claim is processed.

Depending on your plan, your responsibility may include:

Copay or coinsurance
Deductible amounts

Self-Pay Care

Self-pay is available for patients who are not using insurance or who prefer to receive care outside of their insurance benefits.

Fees vary by visit type and length. Current self-pay fees are provided before scheduling so patients can review expected costs before proceeding with care.

For patients who are uninsured or choosing not to use insurance, a Good Faith Estimate of expected charges will be provided when applicable.

Using Insurance or Self-Pay

Some patients use their insurance benefits for psychiatric care, while others choose self-pay based on their individual coverage or circumstances.

If you plan to use insurance, please provide accurate and current insurance information before your appointment so eligibility and billing information can be reviewed. If you plan to self-pay, current fees and expected charges will be provided before scheduling.
